Assuming this configuration worked before and gave suitable fueling, you either have

High fuel pressure, missing vacuum line on pressure reg
High flow reading
Faulty injector or wiring

Maybe leaky injectors.

I don't know really, but 10 seems a bit rich. 12 seems to be the recommended on the street.

Anything below 10 on full boost / throttle is pretty rich, saying that running rich is safer than lean!

Mine run arounds 10.5 - 11.5 on full boost / throttle, wouldn't want to see anything into the 12's just to be safe.

14 - 15 is pretty much spot on for normal driving.

Im running the same VXR injectors, with a 21t and stage 3 chips from Classic Swede!

The first question would be has it always dipped that rich or has it started doing it?
It's quite a lot of injector for a T25 @15psi so your chips fuel map might be causing overfueling on full load.
Failing wideband sensors tend to record rich, I've had this happen once, the engine ran fine but apparently it was in the 9s until I renewed the sensor.
I've had a true rich running problem on load which was my also DIY 3" amm, no black smoke but exhaust stank of petrol, I would deff check that.
Also a loose injector ground on the inlet manifold but that caused rich condition across the board.
Just a few things I've come up against over 5 years of running a bread and butter 250hp setup.
